UNIVERSITY OF CALIFORNIA, SAN DIEGO CONSENT TO ACT AS A RESEARCH SUBJECT

Christine R. Harris, Ph.D. is conducting a research study to explore factors associated with different motivations and attitudes. You are a possible participant in this study if you are at least 18 years of age.

PROCEDURES. If you volunteer to participate in this study, we would ask you to complete a survey. You would be asked to indicate how much you agree or disagree with various statements about personal likes and dislikes, attitudes, experiences, and interests. The total length of time for completion of this survey is approximately 4 minutes.

POTENTIAL RISKS AND DISCOMFORTS. You may find some questions too personal to wish to answer, but if so you may terminate participation simply by closing the browser window.

POTENTIAL BENEFITS TO SUBJECTS AND/OR TO SOCIETY. This research may shed some light on general questions about human behavior.

PAYMENT FOR PARTICIPATION. You will not be paid for your participation in this research study, but if you wish, we will enter you in a drawing for an honorarium/prize as a thanks for participating in our study.

CONFIDENTIALITY. This study is entirely anonymous - no personally identifying information such as email address will be associated with your answers. We will not know or be able to determine your identity.

PARTICIPATION AND WITHDRAWAL. This study is entirely voluntary. If you volunteer to be in this study, you may end your participation at any time without consequences of any kind. If you do not wish to answer any questions, you can simply end your participation by either rerouting your web browser to another website or closing your web browser. You may also refuse to answer any questions you do not want to answer and still remain in the study. The investigator may withdraw you from this research if circumstances arise which warrant doing so. Completion of the survey will constitute consent to participate in the research study.
